Applying for or Renewing MAP and MAP Basic Coverage

Individuals can apply for MAP and MAP Basic coverage or renew their coverage in multiple ways:

• Online using a computer or a smart phone: Apply4MAP.com (English) or InscribaseaMAP.com (Spanish)
• Over the phone: 512-978-8130 (for Eligibility select option 1)
• In-person: Call 512-978-8130 (for Eligibility select option 1) to schedule an appointment
  • Individuals experiencing homelessness do not require an appointment, all other will require an appointment. Call Eligibility for more information and locations.
• CommUnityCare, Lone Star Circle of Care and People’s Community Clinic Eligibility departments can enroll eligible applicants into MAP Basic and submit MAP applications directly to Central Health for enrollment

Applicants for MAP and MAP Basic are required to provide:

• Verification of Travis County Residency
• Verification of income from the last 30 days
• Photo ID
• Verification of U.S. Residency (Non-citizen/non-legal permanent residents can self-declare U.S. residency)

We encourage Providers to review your patient’s MAP/MAP Basic card and encourage all Members whose coverage is expiring within the next 30 days to reapply.

• MAP is not retro-active. Coverage begins on the date Central Health staff determine eligibility.
• MAP Basic coverage can be provided up to 30 days retro-actively
